Feldene (piroxicam) is a non-steroidal anti-inflammatory drug (NSAID) which relieves or prevents inflammation and pain. It works by inhibiting production of prostaglandins, chemicals in the body which contribute to pain, inflammation and stiffness.

Like other NSAIDs, such as ibuprofen or aspirin, Feldene may be used as-needed or continuously to manage chronic conditions. It is most often used long-term to provide relief from arthritis, often allowing patients to live virtually pain-free and resume normal daily activities. On an as-needed basis it may be used as other NSAIDs, to alleviate headaches, toothaches, menstrual pain so forth.

This product is sold under a variety of brand names depending on region, and is also widely available as a generic.

Getting the Most from Feldene

Dosage and administration with Feldene is similar regardless of whether the product is used short-term or long-term:

• Average dose is 20 mg per day.
• 20 mg doses may be taken all at once or divided into two 10 mg doses.
• Do not exceed 20 mg per day without explicit doctor instruction.

If using Feldene as-needed for acute pain:

• Take 10 to 20 mg, ideally at the earliest onset of pain.
• The medication will still provide some relief if taken at later stages, but may not be as effective.
• Repeat after 24 hours if needed.

If using Feldene long-term to treat chronic pain:

• Take 20 mg every 24 hours.
• Alternatively, take 10 mg every 12 hours.
• Benefits will increase for the first 7 to 12 days of daily use.

While similar to other NSAIDs in most regards, Feldene is a bit different in that the medication builds up in the body over the first week or two of use, during which time its pain- and inflammation-reducing properties become increasingly effective. Patients new to Feldene should use the product daily for at least 2 weeks before evaluating results.

Some other tips to ensure treatment is effective and trouble-free:

• Avoid laying down for at least 10 minutes after taking Feldene.
• Take each dose with a full glass of water.
• Taking each dose with food, milk or antacids may prevent stomach upset.
• Avoid taking other NSAIDs during treatment with Feldene. NSAIDs are very commonly found in over-the-counter products, such as cold or flu treatments. Check ingredients carefully, or check with a doctor or pharmacist if unsure.

Some relief is typically felt within 15 to 30 minutes of taking a dose. It may take an hour for full effects to be felt when used as-needed; as mentioned above, it takes up to 12 days of regular use to realize maximum benefit when used long-term.

Duration of treatment in arthritis or other chronic conditions varies. Generally treatment may continue as long as benefits outweigh risks. Regular doctor checkups are advisable in long-term treatment to ensure adverse effects are not developing, particularly in elderly patients or those with major medical conditions.

Precautions & Adverse Effects

Side effects from Feldene are similar to those that may be encountered with any NSAID. Most side effects are mild and not cause for concern, including:

• Upset stomach
• Headache
• Dizziness
• Mild drowsiness

Because NSAIDs are so commonly used and widely available over-the-counter, many people don't realize they carry risk of serious adverse effects. Warning signs which warrant medical attention while taking Feldene, or any NSAIDs, include:

• Difficulty swallowing
• Swelling of ankles, feet or hands
• Tinnitus (ringing in ears)
• Unexplained neck stiffness
• Changes in urine volume
• Severe stomach pain
• Jaundice
• Vision changes
• Extreme fatigue

Some individuals are at greater risk of experiencing severe side effects, most notably those with a history of:

• Allergic reaction to other NSAIDs
• Asthma
• Blood disorders
• Heart disease
• Heart attack or stroke
• High blood pressure
• Liver or kidney disease
• Severe heartburn
• Ulcers
• Bleeding in throat, stomach or intestines
