Kliofem

Kliofem is a daily combination of Hormone Replacement Therapy (HRT) that is taken daily without fail. Kliofem is prescribed to postmenopausal women who have been without menstruation for at least a year. Dosage

Adults: For the first 14 days of the 28-day cycle, apply a 0.05 milligram (mg) Estradiol (Kliofem) patch (Vivelle-Dot®) twice a week (every 3 to 4 days). Then, for the remaining 14 days of the 28-day cycle, apply one CombiPatch® twice a week (every 3 to 4 days).  Children should not use it.

Interactions

Concomitant use of drugs known to stimulate drug-metabolizing enzymes, especially cytochrome enzymes, like anticonvulsants (e.g., carbamazepine, phenobarbital, and phenytoin) and anti-infectives, may enhance the metabolism of progestagens and estrogens (e.g., efavirenz, nevirapine, rifampicin, rifabutin).

Despite their reputation as powerful inhibitors, nelfinavir,  telaprevir, and ritonavir have inducing characteristics when combined with steroid hormones. 

In clinical practice, increased progestagen and estrogen metabolism might reduce impact and alterations in the uterine bleeding profile. Drugs that limit the action of hepatic microsomal metabolizing enzymes, such as ketoconazole, may cause Kliofem's active components to circulate at higher levels.

Due to reduced cyclosporine metabolism in the liver, concomitant use of Kliofem and cyclosporine may result in elevated blood levels of transaminases, cyclosporine, and creatinine.
