MUSE (alprostadil) is an as-needed treatment for men experiencing erectile dysfunction (ED). The product's name, MUSE, is an acronym for Medicated Urethral System for Erections.

This medication is a penile suppository, administered with an applicator into the urethra. As the medication is absorbed, it relaxes muscles and widens blood vessels in the penis, significantly increasing blood flow. Erections develop 5 to 10 minutes after administration and typically last for 30 to 60 minutes.

Treating ED with MUSE

MUSE comes in single-use applicators, administered as-needed 5 to 10 minutes prior to sexual activity. In most cases an erection will occur even without sexual stimulation.

Each applicator has a stem containing a medicated pellet, a protective cap at the tip of the stem, a bulb on top of the stem, and button on top of the bulb which pushes the medication out of the stem.

• Urinate immediately before application; urination will moisten the urethra and ease both insertion of the applicator and absorption of the medication.
• Twist the protective cap off the tip of the applicator.
• Pull the penis upright so the urethra is straight and gently insert the entire stem of the applicator.
• If any pain, discomfort or a pulling sensation is experienced while inserting the stem, gently remove it and try again.
• When the stem is inserted, push the button on top of the applicator and hold it in place for 5 seconds.
• Gently rock the applicator back forth to disengage the pellet from the applicator tip.
• Remove the applicator and gently massage the penis for at least 10 seconds to help the medication dissolve.
• If a burning sensation is experienced massaging the penis for a longer period may help.
• The penis may become warm, red and very sensitive prior to becoming erect; this is normal and not cause for concern.

For best results sit, stand or walk until an erection develops. Laying down will make it more difficult for blood to reach the penis.

This medication does not affect fertility; if trying to prevent pregnancy regular birth control should be used. Some women experience burning or irritation during intercourse with a partner using MUSE; wearing a condom should eliminate the problem.

Do not use this medication more than twice in one day or more than seven times in one week.

Cautions & Side Effects

Most men use MUSE with no side effects. However, there are circumstances in which there is a greater potential for serious side effects, namely in men who have:

• Any major preexisting medical condition, such as heart or liver disease
• Low blood pressure or taking medication to lower blood pressure
• Any blood clotting disorders
• A history of urethra or bladder infections
• A current infection of the bladder, urethra or penis
• Scar tissue on the penis or urethra
• Penile implants or physical abnormalities such as curvature or a narrow urethra

Drug interactions are primarily a concern with medications involving alteration of blood pressure or that change the way blood clots, such as warfarin.

A condom should be used during sex with a pregnant partner while using MUSE; though unlikely, it does present a potential risk to the health of the fetus.

The most common side effects are mild and pass quickly. If symptoms are severe or persistent, seek medical attention:

• Headache
• Dizziness
• Low blood pressure, fainting
• Minor bleeding from the urethra
• Pain in the penis, testicles or groin area
• Prolonged erections
• Urinary tract infection

Taking too much MUSE can cause very serious adverse effects. In case of accidental overdose, seek emergency medical help even if no symptoms are present.
