Metformin has been the go-to medication for type 2 diabetes (diabetes mellitus) for decades. However, its mechanism of action is yet to be understood. 

Recent studies suggest that the drug effectively targets abnormal and normal metabolic signaling differences. Metformin does this to supplement its glucose-lowering prowess. Learning how Metformin’s mechanism of action can help in the development of novel treatments.

What is Metformin’s Mechanism of Action?

Metformin’s mechanism of action occurs when cellular energy metabolism is altered. The drug works by exerting its glucose-lowering power and preventing more absorption of sugar into the body. It also inhibits mitochondria and weakens protein kinase A and cAMP, and responds to high sugar levels.

When stimulated, AMP-activated protein kinase causes insulin sensitivity. As a result, insulin levels peak, and the body becomes more sensitive to insulin to reduce blood sugar levels.

How it Occurs

When an immediate-release dose of Metformin is taken orally, 70 percent of it gets absorbed in the small intestines. The remaining bit passes through the colon and later on gets eliminated as fecal matter. 

Metformin is discharged out of the body through urine. Surprisingly enough, it doesn’t contain any by-products of the metabolism process. 

Even more, oral Metformin tends to concentrate in the bladder, kidneys, liver, and intestines. This is because these organs play a role in the metabolism of Metformin in the body.

Metformin Intolerance

Metformin therapy is often associated with some side effects for patients who continue taking the drug and those who discontinue the medication. However, the mechanism of action behind the side effects is still unclear.

The side effects may reflect the high concentration of the drug in the intestines. In this case, the drug may disperse slowly until the concentrations go down. If they don’t reduce, a patient may develop an intolerance for the drug

Metformin intolerance can also be mediated by serotonin stimulation. In this case, the enterochromaffin cells will release serotonin to counter the effects of the drug.
